Scoliosis and the Human Spine:  A critical review of clinical approaches to the treatment of spinal deformity in the United States, and a proposal for change

In “Scoliosis and the Human Spine”, Dr. Hawes states,”The Schroth Treatment program is based on the principle that spinal deformity comprises a postural disorder and that patients will benefit from correction of that disorder. The Schroth program is based on the assumption that patients, including children are capable of understanding the clinical aspects of their own deformity and are willing to take an active role in their own health, if given the education and tools to do so.”

Dr. Hawes, regarding exercise based approaches to treatment for scoliosis and the prevailing opinions in twentieth century research:

“It is disturbing and misleading that so many authors have implied or stated that exercises have proven to be ineffective in the treatment of idiopathic scoliosis. To be fair, some of the statements are worded carefully, such that, strictly speaking, they are not false. For example, it seems to be true that ‘most experts agree that exercise alone will not affect the progression of a structural scoliosis’ (Cassella and Hall 1991), but who cares what most experts agree if there is no evidence to back up what they think?”

Dr. Hawes Model for Symptom Development in Scoliosis:

According to the model below, “Cardiopulmonary symptoms are a secondary response to fixed torso asymmetry, which inhibits the mechanical function of the human torso. Pain and psychological distress may occur secondary to cardiopulmonary distress, or may be an independent effect resulting from continuous musculoskeletal strain.”

It is for this very reason, that the Schroth method is effective when treating scoliosis. Patients who master and practice the Schroth method learn breathing techniques to mobilize the fixed rib-cage, which then allows for improved pulmonary function.
